To qualify to be seen at WATCH:

1) You must live in Wayne County.  You will be required to show proof of residency.

2) You must not have ANY form of health insurance, to include Medicaid, Medicare, Social Security Disability, VA Healthcare, Blue Cross Blue Shield, Cigna, etc.

Two appointments will be made for you.  

The first will be your New Patient Screening appointment.  This appointment completes all necessary paperwork.  You are required to bring a picture ID, proof of residency, and any medications you are currently taking. 

 

The second appointment will be with a provider (Nurse Practitioner or Physician Assistant) who will address your current medical needs.
